Seeking Case Manager to travel between several clinics, track and document referrals for clinics; Facilitates a daily group for individuals addicted to alcohol, methamphetamine, cocaine, opioids, and other substances. Examples of topics facilitated during group are: applying for benefits, accessing clothing, food, and shelter programs, transitional and permanent housing issues, and transportation.
